Before your dog’s first day of daycamp, we ask that all clients complete the following documentation. These forms allow us to understand your pet’s medical history, temperament, and daily needs to ensure a safe, happy experience.

Required Forms

  1. Client Information Form – Provides your contact details, emergency contacts, and preferred communication method.
  2. Dog Profile Form – Details your dog’s personality, likes, dislikes, energy level, and play style. This helps us pair your dog with compatible playmates.
  3. Vaccination Record Form – Confirms your pet’s current immunizations. We accept digital copies from your veterinarian.
  4. Daycamp and Boarding Agreement – Outlines our safety policies, hours of operation, and care procedures.

REMEMBER:  All vaccines expire, often at different times.  The Wright Pet Daycamp & Boarding keeps a record of all expiration dates and will email you a notice before a vaccine expires.  Your pet cannot rejoin his or her playgroup unless all vaccines are current.

  • Leash, Collar, and ID tags – A quick-release collar is recommended for safety.  A standard 4–6 foot leash (no retractables) for drop-off and pick-up.  Please note that all Leashes, Collars, and Tags are removed before entering the daycare area for the safety of all dogs.
  • Meal or snack – If your dog eats lunch or needs a snack, portion it out in a labeled, resealable container.
  • Medication (if applicable) – Must be in original packaging with dosage instructions. Our staff is trained to administer medications safely.

What Not to Bring

  • Rawhide or long-lasting chews (they can pose choking risks)
  • Glass or breakable containers
